LEARNING DISABILITY

All students with learning disabilities must provide a copy of a psycho-educational assessment completed within the last 4 years.


PHYSICAL OR MENTAL HEALTH DISABILITY

When a request for accommodation relates to a physical or mental-health disability, the student must provide a current report from the appropriate professional and/or medical practitioner, containing a diagnosis of the disability, description of functional limitations, and recommendations for accommodations.

Request for accommodations will not be reviewed until all supporting documentation has been provided to accessibility services and you have met with the coordinator.

All documentation will be kept on file, until it is considered out dated. Students will be informed if and when updated documentation is required.
